How to obtain Damnation

Naxxramas, Dragonblight, Northrend

Set the raid size to 25 players. Clear Naxxramas’s Military Quarter and open the Four Horsemen chest; also check Gluth.
Item source: Four Horsemen; also Gluth. Check loot and bags after the kill; exact item — #40348.
If the item does not drop, repeat after the reset: raids are usually weekly; Normal dungeons can be reset within the game’s limit.

Before starting

A class being listed means it can use this weapon type’s appearance, not that the weapon is useful to it in combat.
Random loot: one run does not guarantee the item.
